Xanthelasma & the Search for a Handy Man

This patient has what is known as Xanthelasma (description below). You may remember this close snipping and stitching from my Halloween Tricks and Treats video.

This patient has Xanthelasma Palpebrarum (Xanthelasma, for short). It is the most common type of xanthoma, occurs on the upper and/or lower eyelids, and are described as soft, chamois colored or yellowish orange bumps. They tend to increase in number, can get larger over time, and are permanent unless they are surgically removed as you see here. It can be seen in people with high cholesterol levels, so when someone has such growths, we usually make sure that their cholesterol levels are normal and under control.
Here I do what is called a “pinch excision”. Eyelid skin is the thinnest skin on the entire body, and after local anesthesia, as long as there is some laxity in the skin, the areas to be removed can be grasped and lifted up with forceps and clipped off with very sharp scissors, as you see here. Then the edges are closed with suture. In fact, sometimes the areas are not sutured closed, but allowed to “heal secondarily” and the cosmetic result is often excellent. Unfortunately, complete removal of the lesions does not guarantee that a person won’t develop new ones in the future.
